Zweck: Bestimmung der Genauigkeit des Gleason-Scores (GS) der Prostatabiopsie im Vergleich zum GS der Prostatektomie; Bestimmung, ob eine Biopsie ein zufriedenstellendes Diagnoseverfahren ist; Angebot einer aktiven Überwachung für Patienten mit niedriggradigem Prostatakrebs. Diese Studie wurde im Tuft Medical Center als retrospektive Kohortenstudie über den Zeitraum von 2007-2010 durchgeführt. Messungen: Gleason-Scores von 6, 7 und 8-10 wurden jeweils als niedrig-, mittel- und hochgradig eingestuft. Die Kappa-Statistik wurde berechnet, um den Grad der Übereinstimmung zwischen Biopsie und Prostatektomie zu bewerten. Die ROC-Kurve wurde verwendet, um die Sensitivität und Spezifität der Prostatabiopsie für verschiedene Gleason-Grade zu bewerten. Außerdem wurde mit Hilfe des Chi-Quadrat-Tests verglichen, ob die Verwendung spezifischer Kriterien für die aktive Überwachung (Johns Hopkins und UCSF) das Niveau der Aufwertung bei Patienten mit niedriggradigem Prostatakrebs verringern kann. Es wurde die Schlussfolgerung gezogen, dass die Genauigkeit der Biopsie GS bei der Vorhersage der Prostatektomie GS stark eingeschränkt ist und daher die Biopsie kein ausreichendes diagnostisches Verfahren ist, um eine aktive Überwachung anzubieten.
