Die MID kann definiert werden als 'eine Philosophie der professionellen Pflege, die sich mit dem ersten Auftreten und der frühestmöglichen Heilung von Krankheiten auf mikroskopischer (molekularer) Ebene befasst, gefolgt von einer minimalinvasiven und patientenfreundlichen Behandlung zur Behebung irreversibler Schäden, die durch eine solche Krankheit verursacht wurden'. Die MID umfasst die folgenden Grundsätze: Frühzeitige Kariesdiagnose und Bewertung der Kariesaktivität, Klassifizierung der Kariestiefe und des Kariesfortschritts anhand von Röntgenbildern, Bewertung des individuellen Kariesrisikos (hoch, mittel, niedrig), Eindämmung aktiver Läsionen, Remineralisierung und Überwachung nicht kavitierter Läsionen, Einsetzen von Restaurationen bei Zähnen mit kavitierten Läsionen unter Verwendung von Minimalhohlraumdesigns und Bewertung der Ergebnisse der Krankheitsbewältigung in vorher festgelegten Intervallen. Die Ausrottung der Krankheit und die regelmäßige Nachsorge bilden den Eckpfeiler der MID.
