Kognitive Funktionsstörungen bei Typ-2-Diabetes mellitus gehen mit einer Abnahme der psychomotorischen Geschwindigkeit, der Frontallappen-/Exekutivfunktion, des verbalen Gedächtnisses, der Verarbeitungsgeschwindigkeit, der komplexen motorischen Funktionen, des Arbeitsgedächtnisses und des unmittelbaren Erinnerungsvermögens einher. Obwohl Typ-2-Diabetes mit mehreren anderen vaskulären Risikofaktoren für Demenz in Verbindung gebracht wird, wurde Diabetes selbst als unabhängiger Risikofaktor für kognitive Beeinträchtigungen und Demenz in Betracht gezogen. Es handelte sich um eine prospektive Beobachtungsstudie zur Beurteilung der kognitiven Funktion von Diabetikern, die mit verschiedenen Klassen oraler Hypoglykämika mit oder ohne Begleiterkrankungen in der Ambulanz des Govt. Headquarters Hospital, Nilgiris und verschiedenen Tertiärkliniken in Coimbatore behandelt wurden. Es wurden 400 Patienten untersucht, von denen 325 Patienten die Ein- und Ausschlusskriterien erfüllten. Die vorliegende Studie zeigte, dass Patienten, die Dipeptidylpeptidase-4-Hemmer zusammen mit Metformin einnahmen, einen besseren globalen kognitiven Score aufwiesen als Patienten, die Alpha-Glucosidase-Hemmer, Thiazolidindione oder Sulfonylharnstoffe zusammen mit Metformin einnahmen.
