O timolipoma é um tumor raro, de crescimento lento e benigno, que pode ocorrer em qualquer idade, independentemente do sexo. Esta lesão desenvolve-se em 81% dos casos no mediastino anterior e inferior. A sua patogénese continua a ser mal compreendida. Apesar do seu grande tamanho, o timolipoma permanece assintomático durante muito tempo e é geralmente descoberto incidentalmente numa radiografia do tórax. A sua expressão radiológica, embora bastante evocativa nas diferentes modalidades de imagem, pode por vezes colocar certas dificuldades de diagnóstico. A RM, graças à sua melhor resolução de contraste, permite compensar as insuficiências da TC e distinguir o timolipoma de outras massas gordurosas como o lipoma, teratoma e lipossarcoma. A RM também desempenha um papel importante na avaliação da resecabilidade e parece ser mais eficaz na detecção de sinais preditivos de malignidade. O diagnóstico de certeza baseia-se no estudo anatomopatológico da peça cirúrgica. O único tratamento curativo é a remoção cirúrgica, geralmente por esternotomia. A evolução é geralmente espectacular, marcada por uma clara melhoria clínica e por um baixo risco de recidiva.
