A doença pulmonar obstrutiva crónica (DPOC) é uma doença muito comum, cuja evolução é pontuada por episódios de exacerbações agudas (EA). O objectivo deste estudo era investigar as características epidemiológicas, clínicas, paraclínicas, terapêuticas e evolutivas das exacerbações agudas da DPOC em pacientes hospitalizados num departamento pulmonar durante um período de 9 anos, e procurar factores preditivos de gravidade. Foram incluídos setenta pacientes com DPOC e AE. A idade média dos nossos pacientes era de 66,1 anos, com uma clara predominância masculina. A intoxicação por fumo foi observada em 68 pacientes (97,1%). Durante o período de estudo, 165 EA requereram hospitalização no departamento pulmonar e 181 EA foram tratados como pacientes externos. Cento e vinte e nove episódios de EA hospitalizada (78,2%) corresponderam à definição de uma exacerbação grave. Factores correlacionados com exacerbações graves foram o tabagismo activo, hiperleucocitose, PCR elevado, hipoxemia, hipercapnia e acidose respiratória.
