Introducción: La Enfermedad Renal Crónica (ERC) actualmente se le considera como una enfermedad catastrófica a razón del incremento de casos, afecta a un porcentaje importante de la población y está relacionada con fenómenos o enfermedades de alta prevalencia como la Hipertensión Arterial y Diabetes Mellitus.Objetivo: Contribuir al diagnóstico precoz de la ERC en población riesgo, pertenecientes CMF 19-47 del Policlínico Universitario, Capitán Roberto Fleites. Conclusiones: Predominó un nivel de conocimiento bajo, se logró diagnosticar en la Atención Primaria de Salud un número de pacientes en estadios iniciales de la ERC, a través de los resultados de creatinina sérica y cálculo del Filtrado Glomerular, se demostró la relación de la Hipertensión Arterial y la Diabetes Mellitus como principales entidades asociadas al desarrollo de la ERC.
