O papel do microbiota intestinal é crucial para a manutenção da saúde humana. Vários estudos afirmaram o papel do microbiota intestinal na gestão de perturbações metabólicas e mentais, bem como de doenças do sistema cardiovascular e do sistema nervoso autoimune. Em geral, o microbiota intestinal está envolvido na atribuição de funções específicas no metabolismo de medicamentos e nutrientes do hospedeiro, na proteção contra determinados agentes patogénicos, na manutenção da integridade estrutural da membrana da mucosa intestinal e na imunomodulação. Geralmente dividem-se em Bacteroidetes e Firmicutes. O microbiota intestinal influencia muitas áreas da saúde humana, desde a imunidade inata até ao apetite e ao metabolismo energético. Alguns estudos investigaram recentemente o papel vital do microbiota intestinal no metabolismo do colesterol, possivelmente através de cinco mecanismos diferentes: alteração da proporção de ácidos biliares livres para ácidos biliares conjugados; fermentação da fibra alimentar para produzir ácidos gordos de cadeia curta que podem inibir a síntese do colesterol; expressão genética mediada pelo microbiota para o metabolismo do colesterol; conversão do colesterol em coprostanol de baixa absorção; e redução do colesterol no sangue através da inibição dos lipopolissacáridos.
