L’apnea ostruttiva del sonno (OSA) è un disturbo cronico comune, caratterizzato dalla ripetuta interruzione parziale o completa del flusso d’aria, associata a desaturazione dell’ossiemoglobina e a un maggiore sforzo per respirare, a un sonno insufficiente, a sonnolenza diurna, a disturbi dell’umore e a stress. I soggetti con vie aeree ristrette e/o anomalie cranio-facciali possono avere un rischio maggiore di OSA. È classificata come centrale, ostruttiva e mista. Le caratteristiche scheletriche comprendono mandibola retrognatica, arco mandibolare stretto, retrognatismo dell’arco mascellare e mandibolare, posizione dell’osso ioide, ecc. La diagnosi del russamento e dell’apnea notturna richiede diverse modalità come la riflessione acustica, la fluoroscopia, la rinofaringoscopia, la cefalometria e la tomografia computerizzata. In base alla diagnosi, l’OSA può essere gestita con modifiche del comportamento, farmaci, terapia a pressione positiva delle vie aeree, apparecchi orali come posizionatori mandibolari anteriori, dispositivi per trattenere la lingua, dispositivi per il sollevamento dell’ugola del palato molle e interventi chirurgici.
